How to add translations and electronic signatures to any ParentSquare form or permission slip, so families can receive forms in their preferred language and sign them online from any device. Translations follow the district's language settings and can be auto-generated or manually reviewed before sending. Electronic signatures let parents sign once and ParentSquare records the signer's name, date, and timestamp for audit purposes.
Admin, Teachers, Group Owners, Room Parents, and anybody with permission to post at districts and schools. Admins and those with individual user permission for Forms can create forms in the Forms Repository.
This article covers optional features you can add when creating any form, whether it is a one-time form in a post or a reusable form in the Forms Repository. Forms must be enabled for your school. Admins can contact ParentSquare support to enable Forms.
Add Translations to a Form
By default, Forms are sent to users in English. When creating a form, you can add translations so parents receive the form in their preferred language. This helps families who speak a language other than English fully understand and respond. For more information, see How Translations Work in ParentSquare.
Add Translation to a One-Time Form in a Post
- From Home, click New Post. Fill out To and Subject fields and Description (optional).
- Under Post Add-ons, select Forms/Permissions.
- Select Create a new form and create the form in English.
- After filling in the details of your form, click Edit Translations under the post description.
- Edit the translated text on the appropriate language tab, or use the machine-translated version. To add or remove language tabs, see Settings for Language Translations.
- When ready, click Post Now.
Add Translation to a Form in the Forms Repository
- From Add-ons in the top menu, click Forms/Permissions in the left sidebar.
- You will see your Forms Repository. From the New Form drop-down menu, select Build your own, Duplicate existing form, or Build From ParentSquare Template. When you duplicate an existing form or template, the translate option will display.
- Create your form in English.
- After filling in the details for your form, select Translate at the bottom.
- Edit the translated text, or use the machine-translated version.
- When ready, click Publish. Users can now use this form (including translation) when they send a new post.
- From the Forms Repository, you can sort forms by Language.
Use Electronic Signatures in a Form
This section explains how electronic signatures work in ParentSquare so you can decide whether to turn them on for your school.
Sign a Form Using an Electronic Signature
Note: these steps apply to forms created after April 13, 2023.
- The user receives an email or text link to Complete Form and Sign.
- Click the link to go to a signature page within ParentSquare. If not already logged in, the user is required to log in to view the form and sign.
- Fill in all required form fields and click Review and Sign.

Mobile App View: - Review the completed form (as a PDF file).
- Click Back to Form (to edit form responses) or Sign Document.
- Review the disclaimer and type the full name to be used as the electronic signature. This name MUST exactly match the name used to log into ParentSquare (the user account name).
- Click I Approve or I Do Not Approve.

District or School Customized Text
Each school or district can request an electronic signature at their own discretion and can provide customized consent wording for giving permission or indicating agreement. Form approval can ask for permission, approval/agreement, or verification.
Choose the Signature Type
If the form requires a signature, click Form requires signature and select from the options:
- For giving permission to child* - provides permission for the student to participate in the specified activity outlined in the form.
- For indicating approval/agreement - provides consent and/or understanding of the terms and conditions outlined in the form.

For verification - provides authentication of the provided information.
*Requiring a signature for permission for a child will automatically include requiring student information in order to collect which student the signature applies to.
ParentSquare can customize your consent text (click Show Disclaimer) if you have specific legal language needed for your form.
Under the ESIGN Act, electronic signatures are defined as any electronic symbol reasonably related to a contract created with the intent to sign that contract. This can range from electronically checking a box that indicates acceptance, to typing one's name into a data field, to hitting an "Agree" button.
View the Forms Report
The Forms Report can be viewed by Admin and the post author. You can see who has signed the form, download the report, and view and download the consent form certificates as PDF files.
- Locate your post, or choose from the Forms/Permissions box on the right.
- Click the gear icon or select from the post detail page. Click Forms > View Report.

Export CSV, Print, or use the checkbox next to names, then click Download and choose Download or Download with Certificate.
- To the far right of a name in the Action column, select Download or Delete.
- The signature form, with or without the certificate, is a PDF file you can save for your records. The Signature Certificate includes who signed and an audit log with date and time.
Parent View
Parents can view and download their responses.
- At the bottom of the post, click Complete Form.
- If they have already signed the form, they can click View & Download.

Note: Students (if StudentSquare is enabled) can fill out a form, but not when a signature is required or if student information is required. Deselect these boxes when creating a form that is sent to students.
